This holiday season, Hearts & Minds is wishing you wonderful, festive holidays. We hope that this will be a time of celebration for you and your family, and we thank you for your support and enthusiasm for what we do.

We also hope that you will take some time in the next few weeks to consider supporting Hearts & Minds Activity Center by donating to the Annual Board Matching Campaign

This year, from December 1st, 2024, to January 6th, 2025, our Board of Directors has generously committed to match all gifts received by Hearts & Minds, up to $33,000. Between these two dates, all donations to Hearts & Minds Activity Center will be DOUBLED.

Hearts & Minds Activity Center addresses the urgent, growing need for dementia care in Santa Clara County through our active and therapeutic activity program serving individuals with every stage and level of dementia. We go above and beyond with staff who are trained in dementia care, extended service hours to accommodate working caregivers, attention and assistance with personal care and ensuring that everyone served has an individualized, personal plan of care. We also host caregiver education and support connecting our families with essential resources to manage their dementia care journey and the respite they receive from their caregiving work is truly lifesaving for the caregiver. Your support will allow us to continue to provide these vital services for our families and our community.

In 2024 we provided 12,798 days of respite for 137 persons living with dementia and 1,791 hours of intergenerational services for our seniors and children in our childcare and, this year, we are on track to surpass these numbers. 57% of persons we serve are low to very low income and we would not be able to meet their needs without the generous gifts of support from you.
